Genesis HomeCare & Hospice

Home Health Services

Whether recovering from an injury, surgery or managing a chronic illness, home health care many times is the perfect solution. The team at Genesis HomeCare is able to provide patients with needed medical care in the comfort of their home allowing patients to remain independent and still receiving the care they need.

One of the determining factors for home care eligibility is homebound status. The patient is considered homebound if he or she experiences normal inability to leave their home. The patient is allowed brief and infrequent absences from the home for non-medical reasons, like attending church, and still considered eligible for home care services.

Patients include seniors, individuals with disabilities, chronic illnesses and those recovering from surgery or accidents.  These individuals may live alone or with a family member but because of their health, need assistance and support to remain independent in their home.

Individual patient needs vary and a registered nurse is assigned to visit the home for an assessment and evaluation of the client's needs and to implement a plan of care for that individual.

Some of the common diagnoses that receive home health care services include:

  • Burns
  • Congestive Heart Failure
  • Chronic Airway Obstruction
  • Diabetes
  • Dysphagia
  • Depression
  • Incontinence
  • Hypertension
  • Osteoarthritis
  • Osteoporosis
  • Circulation problems
  • Pressure ulcers/open wounds
  • Knee or Hip replacements

Skilled Nursing Care

Our team of registered nurses provides direct care to patients in their own home, observing and evaluating the patient's progress, while offering instruction to the patient and his or her family caregiver to continue care between visits. Our patient care coordinator will assess patient condition on initial visit and develop a plan of care that will be shared with all team members. The patient care coordinator also monitors the patient's progress and regularly communicates with the physician, pharmacist and other team members involved in the patient's care.

Social Services

Our team of social workers is able to help patients access any needed medical supply assistance or other local programs that will positively impact the patient's health and quality of life. We are also able to provide financial and emotional counseling to our patients.
